WHAT IS QUALITY?
Quality is of the utmost importance as it pertains to the healthcare documentation process. As the beginning and end of a patient’s medical history, there must be accountability to the: accuracy, accessibility and overall value of the medical record for understanding patients’ symptoms, treatments, and progress.
Trust But Verify
Without quality, the documentation doesn’t exist. Peachtree Transcription’s quality procedures are documented and communicated throughout the Organization. The results of a quality check are verifiable. Thus we make our Quality Reviews available to our clients as needed.
Defining The Problem
Defining an error leaves no room for ambiguity when it comes to Quality Reviews. Peachtree strictly adheres to the AHDI principals and definitions for Quality Reviews.
Accountability
Peachtree’s quality review methodology and formulas, used in QA calculation, come directly from the AHDI Healthcare Documentation Quality Assessment and Management Best Practices Guidelines. We keep the process transparent and verifiable by all parties.
Reliable Results
Peachtree Transcription can provide reliable results because they are capable of being reproduced. Defining the different types of errors and assigning them a point value allows us to produce quality checks consistently. Therefore the same report graded by three different individuals will result in the same score.
Collaborating For Success
Collaboration between Peachtree Transcription and the persons directly responsible for the electronic health record is how we are successful. Peachtree’s open lines of communication allow physicians to ensure reports are formatted and transcribed to their specifications. Also, bi-weekly meetings between Peachtree and our clients enables open and honest discussion of errors and formatting preferences.
Quality Guidelines
- Promptly transcribe complete and accurate details into the electronic health record.
- Patient documents are accessible to the intended users within the requested time frame.
- Quality Audits are statistically random, and require the audible file to validate the integrity of the electronic health record.
- Quality Audits for all services (i.e., Speech Recognition, Manual Transcription and Virtual Scribes).
- Quality Audits by well-trained and highly experienced individuals that show no bias when scoring assessments.
- Error values must accurately reflect the definitions provided herein.
- Communication about the error identification and corrective action help the team become better documentation specialists.
- Reference materials are regularly updated and include account specifications for each client.
